Getting Referred Is Simple — Here's What to Bring

Most patients arrive to us via a referral from their general dentist, but self-referrals are also welcome. Here's what helps your first visit go smoothly.

  • Referral letter or note from your dentist (if applicable)
  • Recent dental X-rays or imaging — your dentist's office can forward these
  • A list of current medications and any relevant medical history
  • Insurance card and photo ID

If something is missing, our office can often request it directly from your referring dentist's office before your appointment.

What Happens During Your First Consultation

1. Check-in & Records Review — Front desk confirms your referral, insurance, and medical history, and collects any imaging you've brought.

2. Clinical Examination — The periodontist examines your gums, teeth, and jaw, checking for signs of gum disease, bone loss, or other concerns.

3. Diagnostic Imaging (if needed) — Additional X-rays may be taken on-site to get a complete picture of your periodontal health.

4. Discussion & Treatment Plan — The periodontist explains findings in plain language and outlines recommended next steps, timelines, and options.

5. Questions & Scheduling — You'll have time to ask questions before scheduling any follow-up treatment.

For patients without insurance, or with out-of-pocket costs, our team discusses payment options directly during the consultation so there are no surprises.
